Michigan Community Dental Plan

Michigan Community Dental Plan (MCDP) offers dental services at a reduced fee for people without dental insurance. 

Membership in the Northern Dental Plan is $50.00 per person.

To be eligible, you must:

• have no other dental insurance.
• fall within the following income guidelines:

When you enroll in the MCDP, your first visit will be free at any participating provider and will include x-rays and an oral exam. Additional services will be available to at  reduced fees. You and your dentist will develop a treatment and payment plan that will meet your needs.

To participate in the program:

You may pick  up  an application at any Michigan Community Dental Clinic office, Dental Clinics North location, or at participating health departments and participating private dentist's offices.

Once your application and membership fee have been received and processed, your card will be mailed to you, along with a list of participating private dentists in the area. You will receive your membership card by mail in 2-3 weeks.
