Welcome to the family of patients served by Michigan Community Dental Clinics, Inc.  As a new patient, you'll be joining over 50,000 people whom we have had the privilege to serve from our network of clinics throughout Michigan.

Before we can schedule you for an appointment, we must first register you in our system.  Please call us to speak with a staff member who will assist you.  This call should take no more than 10 minutes, after which you will be instructed to contact the clinic of your choice to schedule your appointment.

Your Responsibilities and Rights as an MCDC Patient

In order to maintain the reduced fees that MCDC is able to offer our patients, it is critical that we make the most effective use of our time.  Missed appointments, late arrivals, and last-minute cancellations prevent us from utilizing our time appropriately, and this affects all of our patients.  To help eliminate these occurrences, MCDC has instituted a no-show policy that will be discussed with you at your first appointment.

In addition, before MCDC personnel can treat you, you must consent to allow this treatment to be performed.

Finally, as an MCDC patient, you have a right to expect that we will maintain the confidentiality of your dental records.

Your First Appointment

When you arrive at the clinic for your first appointment, you will be asked to read some information and provide some personal history:

  • A form confirming that you have read and understand MCDC's No-Show / Cancellation policy, Consent of Treatment acknowledgment, and your right to privacy notification.
  • A health history that we will scan into your permanent patient record.

For Non-Medicaid, Low-Income Uninsured Patients

The Michigan Community Dental Plan (MCDP) and Northern Dental Plan (NDP) offer dental services at reduced fees to patients without dental insurance.  The Plan available to you will depend on the clinic at which you schedule your appointments and receive your services; see the list below.

Membership in each Plan is $50.00 per person.  This one-time charge will enroll you in the Plan for as long as you are an MCDC patient.

The fee for your initial appointment is included in your Plan membership.  This initial appointment will include an oral exam and X-rays.  You and your dentist will develop a treatment plan to meet your specific needs.  Additional services will be available at reduced fees.

You can pick up an application at any MCDC clinic, participating health department offices, private dentist's offices, or by clicking on either the MCDP or NDP link below.  Please mail your application and payment to the address included on the application form.  Once your application has been received, your membership card will be mailed to you within 2 to 3 weeks, along with a directory of participating private dentists in your area.
